Athletic Forms


Each of the following forms must be brought to the office when your child chooses to participate in a sport:
Athletic Physical Card (signed and dated by a physician) – OR –
Alternate Year Physical Card

(Verification needed by the office that your student does not need a physical that year. This can be done via a phone call to the office at 893-6911.)
2016-17 WIAA Athletic Eligibility Form (Signature page needs to be signed, dated and returned.)
PHS Athletic Code (Signature page needs to be signed, dated and returned.)
Concussion Form (Signature page needs to be signed, dated and returned.)

NOTE: To obtain their initial blue card, a student must bring all four forms to the office.


Athletic passes available

Make it more economical for you, your family and your kids to attend Plymouth High School sporting events by purchasing an athletic pass. Passes for all home games can be purchased and are good from September through May. These nontransferable passes cost $30 for the adult home pass and $10 for the student pass.

Passes can be purchased in the Plymouth High School office during normal business hours, or at any sporting event where admissions are collected.

Single-game admission prices: $4 for adults, $2 for students in 5th through 12th grades, $1 for children in kindergarten through 4th grade. Plymouth High School student admission is paid through the yearly activity fee; however students must have their ID with them.
