Athletic forms

ALL of the following must be brought to the PHS office before a student can practice:

• Athletic Physical Card / Evaluación Física Previa A La Participación (Page 4 signed and dated by a physician) – OR – Alternate Year Physical Card (signed and dated by a parent/guardian)
• WIAA Athletic Eligibility Form / Boletin Informative de Elegibilidad para Deportes de Escue la Secundaria (Signature page needs to be signed and dated by athlete and parent/guardian.)
• PHS Activity/Athletic Honor Code signature page (Needs to be signed and dated by athlete and parent/guardian.) - Full Activity/Athletic Honor Code may be found in the PHS Orange Guide student handbook, beginning on Page 27
• Concussion/Cardiac Form (Signature page needs to be signed and dated by athlete and parent/guardian.)
• Release and Waiver of Liability Form (Needs to be signed and dated by parent/guardian.)
• $85 PHS Social & Extracurricular Fee

Game admission fees

Regular-season fees

• Preschool & younger: free
• Grades K-4: $1 cash or $2 online
• Grades 5-12: $2 cash or $3 online (free for PHS students with school ID)
• Adult: $5 cash or $6 online
• Senior (age 62+): free
Please note: Playoff games have different pricing, set by the Wisconsin Interscholastic Athletic Association.

GoFan online ticketing

Plymouth High School offers online ticketing via GoFan for all home athletic events, which allows fans to purchase tickets in advance or at the gate using a credit card.

Please note: We continue to accept cash at the gate.

Athletic passes

Passes, good for all regular-season home games this school year, can be purchased in the PHS office during normal business hours or at any sporting event where admissions are collected.

These nontransferable passes cost $30 for adults and $10 for students. (Please note that admission is free for PHS students with school ID, children younger than kindergarten, and senior citizens older than 62.)
