PHS fall sports begin soon

Posted July 18, 2022

Recognizing the value of extracurricular activities, Plymouth High School encourages students to join its athletic teams this fall.

How to participate

Any student-athlete who would like to participate in a PHS sport needs to submit the following items before practicing with the team:
• Completed Athletic Physical Card (signed and dated by a physician) – OR – Alternate Year Physical Card (signed and dated by a parent/guardian)
• Signature page of WIAA Athletic Eligibility Form signed by athlete and parent/guardian
• Signature page of PHS Activity/Athletic Honor Code signed by athlete and parent/guardian
• Signature page of Concussion/Cardiac Form  signed by athlete and parent/guardian
• $85 PHS Social & Extracurricular Fee

Printed forms are available in the PHS office (enter Door 1G), which is open 7 a.m. to 2:30 p.m. Monday-Friday this summer. If you have questions on which Physical Card you need, contact the PHS office at 920-893-6911, option 3.

Once the items above are turned in at the high school office, a blue permit-to-practice card will be issued, beginning July 20. An athlete will not be allowed to practice until that blue card is given to the head coach.

The annual Athletic Code Meeting will be held Wednesday Aug. 31 in the PHS Auditorium, beginning at about 7:30 p.m. after PHS Freshman and New Student Orientation Night. Freshmen and first-time athletes and their parents/guardians are required to attend.
