Registration Information

The school district is required by law to keep a current census of children from newborn to age 20 in the district. All families are asked to complete the Census Form each year.

Any new student of the district must complete an affidavit of residence in the district office located at Plymouth High School prior to registering at a particular school.




4k & 5k Registration

The Plymouth School District offers 4K and regular kindergarten. Children must be 4 years old by Sept. 1 for the 4K program or 5 years old by Sept. 1 for the 5K program. A birth certificate is needed for proof of age.

Parents registering for 4K need to complete a registration form (front and back), a Home Language Survey and a Placement Information Sheet. Parents registering for 5K need to complete the registration form (front and back).

Elementary Reassignment

The Plymouth School District has a designated attendance area for each elementary school. A current District practice (Elementary School Reassignment Program) allows parents of elementary students to request that their child be assigned to another school.

Step 2: Netbook Information
Upperclassmen will be issued their netbooks at orientation the evening of Wednesday, August 21st. More information will be supplied in the August Newsletter Packet later this summer. Freshmen will be issued their netbooks during the first week of school. Please read the following documents regarding internet usage at Plymouth High School. STUDENTS MUST BRING the completed Netbook Form, insurance payment (if you want to participate in the school district protection plan) and a case for the netbook. (NOTE: Original cases do not need to be used. Inexpensive cases can be purchased at most retail stores.)

Step 3: 2013-2014 Parent/Student Directory
Parents interested in being a part of the Parent/Student Directory click on the link below. Deadline to read and complete the survey is JULY 13, 2013. You will receive a copy of the directory based on how your questions were answered. If you are asking for a printed version of the directory you will need to bring $1.00 to the office and pick one up. We will email parents when the directory is complete. (NOTE: Currently we are looking for an individual to lead the Parent Network. Although we are collecting the data for the directory it will only be printed if someone takes over this leadership role.)

Step 4: Athletic Forms - DO NOT RETURN UNTIL AUGUST 1, 2013 OR LATER
Each of the following forms must be brought to the office when your child chooses to participate in a sport:
Athletic Physical Card (signed and dated by a physician) – OR –
Alternate Year Physical Card

(Verification needed by the office that your student does not need a physical that year. This can be done via a phone call to the office at 893-6911.)
WIAA Athletic Eligibility Form 2013-2014 (Signature page needs to be signed, dated and returned.)
PHS Athletic Code (Signature page needs to be signed, dated and returned.)
Concussion Form (Signature page needs to be signed, dated and returned.)
NOTE: To obtain their initial blue card, a student must bring all four forms to the office. In addition, a Concussion Form signature page is needed for each additional sport.
